- Nguy cơ thai nhi mắc bất thường nhiễm sắc thể (ví dụ hội chứng Down) tăng dần theo tuổi mẹ (2).

Pregnancy and childbirth are among the most important events in a person's life, for each family and for society as a whole. A healthy pregnancy and a safe birth for both mother and baby is what every family, and every healthcare team, hopes for.
Most pregnancies go smoothly. Even so, serious complications still occur, some of them life-threatening for the mother or baby during pregnancy or after birth. Many of these risks can be identified and reduced before pregnancy even begins. Good preparation before conception, combined with timely monitoring and management of high-risk situations, is the most effective way to protect the health of mother and child.
Below are evidence-based recommendations for women who are planning a pregnancy.
1. What is a favorable age for pregnancy?
- A population-based case-control study (Pethő et al., 2023) found that women who conceived between the ages of 23 and 32 had the lowest risk of having a baby with non-chromosomal congenital anomalies (1).
- The risk of fetal chromosomal abnormalities (such as Down syndrome) rises steadily with maternal age (2).
- Female fertility declines gradually but significantly from about age 32, and more rapidly after age 37 (3).
If you plan to have children, consider not postponing too long. Women under 35 who have been trying to conceive for 12 months, or women 35 and older who have been trying for 6 months, should see a doctor for an evaluation (3).
2. Adopt a healthy lifestyle
Don't smoke, and avoid secondhand smoke
Smoking during pregnancy increases the risk of fetal growth restriction, placenta previa, placental abruption, premature rupture of membranes, preterm birth and low birth weight. Children born to mothers who smoked during pregnancy are at higher risk of asthma, infant colic and childhood obesity (4). Marijuana may cause similar harm and should be avoided completely (4). Quit before you conceive, and encourage family members to quit as well to avoid secondhand smoke.
Avoid alcohol
No amount of alcohol is known to be safe during pregnancy. Drinking while pregnant can cause fetal alcohol spectrum disorders (4), including:
- Physical defects: central nervous system abnormalities, growth deficits and facial malformations.
- Intellectual and behavioral problems: cognitive impairment and behavioral disorders.
Because many women don't know they are pregnant for the first few weeks, it is best to stop drinking as soon as you start trying to conceive.
Maintain a healthy weight
Both underweight and overweight or obesity are linked to complications for mother and baby (4). Aim to keep your body mass index (BMI) in the normal range of 18.5–24.9 kg/m². Note that for Asian populations, the World Health Organization suggests lower cut-offs, with a BMI of 23 or above already considered overweight (5).
Formula: BMI = weight (kg) ÷ [height (m) × height (m)]. You can calculate your BMI here: NHLBI – Calculate Your BMI.
Stay away from harmful chemicals
Limit exposure to environmental pollutants, workplace teratogens and endocrine-disrupting chemicals such as plastics containing bisphenol A (BPA), pesticides, lead-based paint and asbestos (4). These substances can affect fertility and cause birth defects. If your job regularly exposes you to chemicals, talk with your doctor.
Be physically active
Adults should get at least 150 minutes of moderate-intensity activity per week (for example, brisk walking 30 minutes a day, 5 days a week), plus 2 days of muscle-strengthening activity each week (6). Physical inactivity increases the risk of heart disease, type 2 diabetes and some cancers.
Get enough sleep
Aim for 7 or more hours of sleep per night. According to a joint consensus statement from the American Academy of Sleep Medicine and the Sleep Research Society, regularly sleeping less than 7 hours a night is associated with weight gain, obesity, diabetes, high blood pressure and impaired immune function (7).
3. Schedule a preconception visit with your obstetrician
Ideally, see your doctor a few months before you plan to conceive. During the visit, your doctor will (4):
- Assess your overall health and obstetric history, and identify and optimize chronic conditions such as high blood pressure, diabetes, thyroid disease or epilepsy before pregnancy.
- Review your current medications: some need to be adjusted or switched before conception. Do not stop any medication without talking to your doctor first.
- Check your immunity and update vaccinations against measles–mumps–rubella, chickenpox (varicella), hepatitis B and others. After receiving a live vaccine (such as MMR or varicella), avoid becoming pregnant for at least 1 month.
- Order appropriate tests, such as screening for HIV, hepatitis B and syphilis, and, when appropriate, genetic carrier screening (for example, for thalassemia).
- Prescribe folic acid: take 400–800 mcg (0.4–0.8 mg) daily, starting at least 1 month before conception and continuing through the first 2–3 months of pregnancy, to reduce the risk of neural tube defects in the baby (8). Women who have previously had a pregnancy affected by a neural tube defect may need a higher dose as directed by their doctor.
Don't hesitate to write down your questions and bring them to your appointment.
